only for purchasing a puppy or placing a waiting list deposit!! 

Adoption forms are to join our waiting list and place a deposit for a future puppy or to claim and put a deposit on a puppy we have available now.  If you are just wanting more information please message 903-280-4058.  
Please be sure to let us know if you plan to pay your total at pickup with a credit card, if prior arrangements are not made we only accept cash at time of pickup.
Your answers in no way affect your puppy adoption, they are only to help us know what information you might need.
